Job Description

Aveanna Healthcare is seeking an In Home Healthcare RN to deliver compassionate, high-quality nursing care to patients in the comfort of their homes. In this role, you will perform comprehensive assessments, administer medications and treatments, manage care plans, and educate patients and families on disease management and home care techniques. You will collaborate closely with physicians, therapists, and the Aveanna care team to promote safety, comfort, and optimal outcomes. Responsibilities Perform comprehensive in-home patient assessments and ongoing monitoring Develop, implement, and update individualized care plans Administer medications, treatments, and therapies safely and accurately Provide wound care, IV therapy, and specialized home health procedures as ordered Educate patients and families on disease management, medications, and home care techniques Document all care accurately in the EMR and maintain regulatory compliance Collaborate with physicians, therapists, and other care team members to coordinate services Monitor changes in patient condition and communicate promptly to the healthcare team Promote patient safety, comfort, and independence in the home setting Participate in case conferences, training, and quality improvement initiatives Required Skills Patient assessment Care planning and coordination IV therapy and medication administration Wound care management Ventilator and tracheostomy care Electronic medical record (EMR) documentation Home health regulations and compliance Patient and family education Infection control practices Critical thinking and clinical decision-making
