Job Description

Job Summary Join our dynamic healthcare team as a Home Health Registered Nurse (RN), where you will play a vital role in delivering compassionate, high-quality nursing care directly to patients in their homes. As a key member of our home health team, you will assess patient needs, develop personalized care plans, and collaborate closely with healthcare professionals to ensure optimal patient outcomes. Your dedication will make a meaningful difference in the lives of patients across diverse settings. Duties Conduct comprehensive patient assessments including vital signs, physiology evaluation, and medical history review to determine individualized care needs. Manage patient care plans by coordinating services such as discharge planning, hospice support, and post-acute recovery strategies. Provide skilled nursing interventions within the home environment, including medication management, wound care, and infection control based on infectious disease care protocols. Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to ensure seamless patient care delivery. Utilize Electronic Health Records (EHR) systems for accurate documentation of medical records, clinical case management notes, and compliance with HIPAA regulations. Educate patients and their families on health management strategies, medication adherence, and safety procedures tailored for geriatrics and pediatric populations. Monitor patient progress through ongoing evaluation of vital signs and clinical indicators; adjust care plans accordingly to meet evolving needs. Experience Proven clinical experience in critical care environments such as ICU or Level II/Level I Trauma Centers; hospital medicine or emergency room experience highly preferred. Demonstrated expertise in managing complex cases involving acute illnesses, behavioral health issues, or infectious diseases across diverse patient populations including pediatrics and geriatrics. Strong background in medical-surgical nursing with familiarity in managing patients with multiple comorbidities in outpatient or post-acute settings. Experience with home health services including discharge planning, medical record management, and outpatient case management is highly desirable. Knowledge of health regulation policies such as HIPAA compliance and utilization management practices to ensure quality standards are met. Skilled in using EMR/EHR platforms for documentation; adept at diagnostic evaluation and managing patients as a nurse case manager within managed care frameworks. Excellent client communication skills with the ability to educate patients on health conditions and collaborate effectively with multidisciplinary teams for optimal outcomes. $55.00 - $57.00 per hour Expected hours: 12.0 - 40.0 per week
