Key Responsibilities:

Deliver in-home nursing care with attention to skilled interventions and patient safety Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly Collaborate effectively with physicians and interdisciplinary care teams to execute individualized care plans Provide wound care, medication administration, health education, and disease management support Communicate patient updates and any significant changes in condition to the clinical team in a timely manner

Qualifications & Experience:

Active Registered Nurse license Minimum of one year recent experience in home health RN role Required proficiency in OASIS documentation Strongly prefer experience with HomeCare HomeBase software Reliable transportation and valid drivers license required Comfortable managing a full-time schedule, including daily visits Whats in It for
