Resident Care Coordinator

Job Description

Resident Care Coordinator We are seeking a compassionate and organized Resident Care Coordinator to enhance the experience of our residents and their families. This role serves as a key point of contact, ensuring residents feel welcomed, supported, and comfortable throughout their stay—from admission to discharge.
Schedule:
Monday-Friday | 8:00 AM - 4:00 PM (Full-Time) Occasional MOD weekend
Pay:
Based on experience and qualifications Responsibilities include but are not limited to: Ensure all open rooms are clean, staged, and move-in ready Coordinate closely with Admissions, Marketing, and Social Services for upcoming admissions and discharges Conduct routine rounds to address resident needs and requests Maintain clean, organized, and welcoming common areas Communicate effectively with residents and families, providing information or coordinating follow-ups Conduct follow-up calls with families ("Caring Partner" calls) Confirm required equipment (oxygen, wheelchair, etc.) is ready prior to arrival Greet new residents and families upon admission Assist with: Inventory of belongings Clothing labeling Orientation to services and amenities Help complete admission documentation with families Coordinate discharge readiness and timing Conduct a brief resident satisfaction survey Coordinate post-discharge room turnover with housekeeping and admissions Qualifications High school diploma or equivalent required; associate degree preferred Experience in healthcare, hospitality, or customer service preferred Strong communication and interpersonal skills Highly organized with attention to detail Ability to multitask in a fast-paced environment Compassionate, patient, and resident-focused attitude
Qualifications:
Previous experience in healthcare helpful, but not necessary. Ability to work under stress and in emergency situations. Why Burlington?
