Liberty Health seeks a compassionate Speech Language Pathologist to provide evidence-based evaluation and treatment for adults with communication, cognitive-linguistic, and swallowing disorders. In this role, you will conduct assessments, develop and implement individualized treatment plans, document progress, and collaborate with an interdisciplinary team. You'll deliver person-centered care, educate patients and families, and contribute to quality, outcomes-focused programs that support behavioral health and complex populations in partnership with public-sector agencies. Responsibilities Evaluate speech, language, cognitive-linguistic, and swallowing disorders using standardized tools Develop and implement individualized treatment plans based on assessment findings Provide evidence-based therapy to diverse adult populations Document evaluations, treatment sessions, and patient progress in clinical records Educate patients, families, and caregivers on communication and swallowing strategies Collaborate with interdisciplinary clinical teams to coordinate care Participate in quality improvement and outcomes-focused program initiatives Ensure services comply with professional, legal, and organizational standards Required Skills Speech and language evaluation Swallowing (dysphagia) assessment and treatment Cognitive-linguistic rehabilitation Treatment planning and documentation Use of standardized assessment tools Interdisciplinary team collaboration Electronic health record (EHR) documentation Patient and family education Behavioral health or neuro-rehab experience Regulatory and ethics compliance in healthcare
