Responsibilities for this job include:

Conducting evaluations and initiating referrals when clinically indicated Formulation of an individualized plan of care focused on patient's functional needs Preparation of written documentation Development of rapport and mutual respect with the patient, patient's family and workmates. Demonstration of appropriate knowledge in providing for the assessment and remediation of cognitive, visual-perceptual skills, speech-language deficits, swallowing dysfunction and/or behavioral parameters Providing ongoing education to patients and/or families
