Job Description

Speech-Language Pathologist (SLP) - Skilled Nursing Facility Position Summary We are seeking a licensed, compassionate Speech-Language Pathologist to join our skilled nursing facility team as a Treating Speech-Language Pathologist. In this role, you will provide direct, skilled speech-language pathology services to residents in accordance with physician orders, individualized plans of care, and applicable CMS regulations. You will report directly to the Director of Rehabilitation and work collaboratively within an interdisciplinary team to help residents achieve their highest level of communication, cognitive, and swallowing function. Key Responsibilities Evaluate residents' speech, language, cognitive-communication, and swallowing (dysphagia) needs and develop individualized treatment plans consistent with physician orders and resident goals. Provide direct, skilled speech-language pathology treatment in accordance with the resident's plan of care and PA speech-language pathology scope of practice. Conduct swallow evaluations and recommend diet modifications, compensatory strategies, and swallow safety precautions in coordination with nursing and dietary staff. Accurately and timely document evaluations, treatment sessions, progress notes, and discharge summaries in compliance with CMS, PA Department of Health, and facility documentation requirements. Communicate resident progress, changes in condition, and treatment recommendations to the Director of Rehabilitation, nursing staff, physicians, and family members as appropriate. Participate in care plan meetings, interdisciplinary team meetings, and Quality Assurance/Performance Improvement (QAPI) activities as assigned. Ensure treatment and documentation support appropriate Medicare Part A/Part B and MDS reimbursement requirements. Supervise and direct Speech-Language Pathology Assistants (if applicable), ensuring care is delivered within their scope of practice. Maintain compliance with infection control protocols, resident safety standards, and facility policies. Educate residents, families, and caregivers on treatment plans, communication strategies, swallow safety, and home exercise programs. Maintain equipment in safe working order and report any equipment concerns to the Director of Rehabilitation. Qualifications Active, unrestricted Speech-Language Pathologist license in the Commonwealth of Pennsylvania. Master's degree in Speech-Language Pathology from an accredited program. ASHA Certificate of Clinical Competence (CCC-SLP) preferred. Experience in a skilled nursing, long-term care, or geriatric rehabilitation setting preferred. Working knowledge of CMS regulations, Medicare guidelines, and
MDS/PDPM
as they relate to therapy services. Strong clinical judgment, communication, and documentation skills. CPR certification required. Physical Demands Ability to lift, push, pull, and/or carry up to 50 pounds. Ability to stand, walk, bend, stoop, kneel, and reach for extended periods throughout the shift. Ability to assist with resident positioning and mobility using proper body mechanics as needed. Manual dexterity sufficient to operate therapy/diagnostic equipment and complete documentation. Visual and auditory acuity sufficient to monitor resident conditions, assess swallow function, and respond to safety needs. Work Environment This position is performed in a skilled nursing facility setting. Pay:
$50.00 - $55.00 per hour
