The nurse, under the supervision of the Case Management Director/Manager/Supervisor, is responsible for overseeing discharges that require a safety evaluation. Their duties include identifying risk factors related to functional limitations and environmental conditions, ensuring compliance with medication management, fall risk, and oxygen safety precautions, and providing necessary education to the members. The nurse must confirm that all needs (e.g., durable medical equipment, referrals, and appointments) are addressed, reporting any uncompleted tasks to the Case Management Team for follow-up. Additionally, the nurse is responsible for documentation using the Hospital to Home Safety Evaluation and Plan of Care, and collaborating with the assigned coordinator to schedule visits, explain the program, and prepare educational materials. This position requires patient home visits.

Pay:

$31.20 - $40.60 per hour
