Opportunity:

The care team includes seven Palliative Care APPs and two dedicated social workers.

• Weekday position with protected administrative time.
• Call Schedule 1:4 weeks with

Hospice:

M-F 5:00 pm

• 8: 30 am, 24-hour coverage Sat and Sun.
