Intensive Case Manager for Workforce Development in the Next Step Program The Chamber Foundation, Inc. Full-Time |
In-Person Compensation:

The Chamber Foundation, Inc. is seeking a dedicated and compassionate Intensive Workforce Development Case Manager to join our team and help individuals overcome barriers, secure meaningful employment, and achieve long-term career success. This position works directly with participants in the Workforce Connections Program, providing intensive case management, employment support, job-readiness services, and ongoing coaching designed to help individuals obtain and retain employment. The ideal candidate is an excellent communicator, highly organized, resourceful, and passionate about helping people reach their goals. This position offers the opportunity to work directly with participants, employers, community organizations, educational institutions, and social service agencies throughout the Hudson Valley.

Employment:
The Chamber Foundation, Inc. Key Responsibilities Participant Engagement & Case Management Work directly with program participants in a professional, respectful, and supportive manner. Respond promptly to referrals from the Department of Community and Family Services (DCFS) and follow established enrollment procedures. Conduct comprehensive intake and job-readiness assessments for new participants. Establish individualized SMART Goals and develop employment plans based on each participant's strengths, needs, and career objectives. Maintain complete and accurate participant files, including required agreements, releases, waivers, assessments, and case documentation. Provide intensive follow-up and support to participants throughout their first six months of employment and beyond as needed. Conduct regular in-person and telephone meetings with participants. Conduct home visits and community-based meetings when appropriate, including school meetings, legal proceedings, appointments, and other situations where additional support is needed. Help participants identify and access supportive, transitional, educational, and community-based services that can help them remain successfully employed. Employment & Job Retention Services Assist participants with job searches, interview preparation, workplace expectations, and employment retention strategies. Maintain regular communication with employers to support successful placements and long-term employee retention. Meet or conduct a telephone interview with the participant's supervisor or designated employer representative within five business days of the participant's employment start date or program enrollment. Maintain employer contact at least twice per month and document all contacts and outcomes. Work with employers and participants to identify and resolve workplace challenges. Respond promptly when an employer or DCFS reports that a participant has missed work or is experiencing an employment-related issue. Provide ongoing support to employers to promote successful employee retention and customer satisfaction. Workshops & Career Development Develop and coordinate an ongoing workshop series addressing topics such as: Job readiness and career development Resume writing and interviewing Workplace communication and professionalism Soft skills development Financial literacy and budgeting Conflict resolution Workplace expectations and problem-solving Career advancement and professional development Identify additional training opportunities and coordinate with organizations such as Dutchess County BOCES, Dutchess Community College, and other workforce and training providers. Community & Employer Outreach Promote the Workforce Connections Program to community organizations, employers, social service agencies, and other referral partners. Develop strong relationships with local employers and community partners. Serve as an ambassador for the program throughout the local workforce development and social service communities. Collaborate with Chamber staff and participating agencies and organizations to expand opportunities for program participants. Documentation, Reporting & Compliance Maintain timely and accurate case notes and participant records in the CiviCore database. Document all participant and employer contacts, services provided, referrals, and outcomes. Submit monthly participant progress reports to DCFS by the fifth working day of the following month. Complete Workforce Connections Participant Evaluations every three months of continuous employment. Assist with required monthly and quarterly program reports. Maintain strict confidentiality of all participant records, communications, and personal information. Ensure all services and documentation comply with program requirements and contractual obligations. Collaboration & Program Support Serve as a liaison between participants, employers, DCFS Employment staff, and other service providers. Communicate regularly with the Workforce Program Manager regarding participant progress, challenges, and program activities. Immediately report significant participant crises or other urgent matters to the appropriate supervisor. Attend scheduled mentoring and program meetings. Participate in community meetings, Chamber meetings, informational sessions, and events as requested. Work cooperatively with Chamber Foundation and Chamber staff and partner organizations. Chamber Foundation Responsibilities As an employee of The Chamber Foundation, the Case Manager may also assist with Chamber activities and events, including: Monthly Contact Breakfasts Business After Hours Ribbon Cuttings Community events Chamber programs and meetings Other organizational activities and special projects as assigned The position will also be responsible for submitting mileage and expense reports in accordance with organizational policies. Expenses outside of approved mileage must receive prior authorization. Qualifications Required Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field. Experience working with individuals facing barriers to employment. Knowledge of case management principles and employment services. Strong communication, interpersonal, organizational, and problem-solving skills. Ability to establish trusting and professional relationships with participants, employers, and community partners. Ability to work effectively with diverse populations and as part of a multidisciplinary team. Strong computer skills and the ability to maintain accurate electronic records. Valid driver's license and reliable transportation, as the position includes community and home-based visits. Preferred Experience Experience working with individuals who may have: Mental health challenges Disabilities Criminal justice involvement Employment barriers Housing, transportation, or financial challenges Other barriers to successful employment Experience in workforce development, vocational rehabilitation, social services, employment services, or community-based programs is highly desirable.
