Job Description

Overview The Clinical Integration and Outreach Liaison lead strategic initiatives that strengthen care coordination, expand community partnerships, and improve patient access across the healthcare continuum. This role bridges clinical operations, population health, and external engagement to ensure transparency and a seamless high‑quality care. The liaison collaborates with physicians, hospitals, community agencies, and internal leadership to drive integration efforts that support organizational goals, quality outcomes, and patient experience. Responsibilities Developing new relationships, community outreach, brand building, setting up facility tours. Building and strengthening relationships between the organization and communities it serves. Developing and implementing outreach strategies to engage with the community and promote the organization's mission. Representing the organization at public events and community meetings. Coordinating volunteer efforts and community service programs. Tracking and analyzing community outreach efforts to improve effectiveness and alignment with relevant organizational goals, clients, and facilities including reporting on urgent or irregular concerns. Attend standing meetings with facility leadership. Key qualifications include a strong understanding of community engagement, project management skills, and the ability to work collaboratively with community organizations. Experience Bachelor's degree in a related field or equivalent work experience is preferred. 2-3 years of experience working in the mental health and substance abuse setting and community outreach. Strong communication, organization and interpersonal skills. Ability to work independently and with a team. Excellent organizational and project management skills. Ability to analyze data and provide reports on community engagement efforts. Proficiency in Microsoft Office and social media platforms. Knowledge of community outreach best practices and principles.
Pay:
$80,000.00 - $100,000.00 per year
