Make a Real Difference in Your Community The Chesapeake Connection provides Community Development Services to adults with intellectual and developmental disabilities in Calvert County and the surrounding area. We are looking for an energetic, compassionate Community Support Facilitator to bring our approved Program Service Plan (PSP) to life in a group setting — leading small groups of the people we support in skill-building, volunteering, recreation, and employment-exploration activities out in the community. This is a hands-on, out-and-about role. You won't be behind a desk: you'll be facilitating group activities in integrated community settings that build social, communication, self-advocacy, and independent-living skills — while honoring each person's choices, preferences, strengths, and pace, consistent with the DDA person-centered practices framework. What You'll Do Implement The Chesapeake Connection's Program Service Plan (PSP) in a group setting, facilitating structured small-group activities in integrated community environments. Lead community-based, volunteer, recreational, and employment-exploration activities that promote inclusion, natural relationships, and valued social roles. Build meaningful skills and foster self-determination for each person in the group, adapting activities to diverse abilities and communication styles. Support individuals during meals and personal-care times with dignity, choice, and safety; provide travel training and transportation within the service. Monitor, document, and report each individual's progress toward their goals per DDA documentation standards; report incidents immediately. Communicate respectfully and collaboratively with families, guardians, CCS coordinators, colleagues, and community partners. Maintain a safe, welcoming, and respectful environment in every interaction. Must-Haves Valid Maryland driver's license with a clean driving record — you'll transport participants in the community. At least two (2) years of direct experience supporting individuals with intellectual and developmental disabilities. Current CPR/First Aid certification. Ability to pass a CJIS criminal background check, as required by the Maryland Developmental Disabilities Administration (DDA). Ability to successfully complete all required DDA training upon hiring, including travel training, community risk-management practices, and person-centered documentation and outcome tracking. High school diploma or GED. Physical capacity for an active, community-based role (standing, walking, and moving across community environments). Nice-to-Haves Bachelor's degree from an accredited institution. Experience in human services, instructional support, or a related field. Demonstrated ability to design and facilitate strengths-based group activities.
