Job Description

Community Engagement Specialist - Full Time, Monday through Friday, with Administrator on-call rotation
QUALIFICATIONS
Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Licensed to drive and with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
SUMMARY OF JOB RESPONSIBILITIES
The Community Engagement Specialist contributes to the development of the agency marketing plan and coordinates community education/outreach opportunities for hospice and palliative care in a manner consistent with the philosophy and policy of Harbor Hospice. This position plans and coordinates marketing and education initiatives, speaking opportunities along with assigning responsibility to team members as deemed appropriate. This position acts as a liaison to identified hospitals, facilities, community partners, physicians, medical health professionals, payers and patients/families and all other referral sources through distribution of educational material, in-services, conferences, regular contacts and other marketing activities. Creates and maintains a favorable public image for Harbor Hospice and raises community awareness regarding end of life choices. This position represents the organization professionally and acts as a key driver of referral volume and patient access. ESSENTIAL FUNCTIONS AND DUTIES
(other duties may be assigned) Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment. According to the agencies Notice of Privacy Practices and HIPAA regulations. Is knowledgeable and functional in computer application including database, word processing, spreadsheets, and e-mail system as required for the execution of the job. Develops and executes comprehensive marketing plans and programs, both short and long range, to support objectives of organization with clinical team and community engagement counterpart to support objectives of the organization. Assists with the development of and managing the marketing operating budget. Oversees development and production of promotional materials. Participates in community events and special events, for the purpose of, promoting Harbor Hospice and the Harbor Hospice Foundation. Produces and evaluates marketing activity statistics and presents to management team. Establishes sense of urgency to meet the needs of patients, referral sources and family to access care and services. Post daily census and runs daily reports. Appropriately request and obtain medical documentation from referral sources and upload into patient's medical record as needed. Identify growth opportunities for growth and process improvement to expand referral networks. Demonstrate knowledge of the key components of the hospice concept and philosophy of care. Participates in agency's strategic planning process. Identifies, coordinates and implements strategies to increase awareness/participation in hospice and palliative care of healthcare professionals including physicians, businesses, faith-based groups, and donors. Facilitates appropriate problem solving with internal and external staff as concerns arise. Collaborates in education of community members and business partners on the hospice philosophy and services of Harbor Hospice. Responsible for carrying out projects and tasks related to the community and public relations needs of the agency. Represents employer during community projects and at public, social, and business gatherings. Achieves budgeted census goals and creates quarterly action plans to achieve and maintain. Serves as Administrator on-call on rotation as assigned.
