Job Description

Health Connect America seeks a Bilingual Family Centered Treatment Specialist to provide in home and community based behavioral health services to children, adolescents, and families. You will conduct assessments, develop and implement family centered treatment plans, deliver evidence based interventions, and coordinate care with schools, community partners, and other providers. Collaborate in a multidisciplinary team, maintain timely documentation, and support crisis stabilization. This role is ideal for mission driven professionals committed to compassionate, culturally responsive care and improving family functioning and safety.
