CRM Supervisor About Spark Wellness Spark Wellness is a Joint Commission-accredited, state-licensed behavioral health and substance use disorder treatment center located in Oregon City. We provide PHP/IOP and outpatient services to adults seeking recovery and healing through evidence-based, trauma-informed, and person-centered care. We are seeking a Case Manager and Certified Recovery Mentor (CRM) who is passionate about supporting individuals on their path to recovery, connecting them with community resources, and fostering long-term wellness in alignment with
OAR 309-019 , MHACBO
standards , and Joint Commission Behavioral Health accreditation . Position Summary The Case Manager and Certified Recovery Mentor (CRM) serves as both a mentor and guide for individuals in recovery while providing clinical case management as a Certified Alcohol and Drug Counselor-Registered (CADC-R) .This position combines compassionate peer mentorship with professional case management to ensure patients receive comprehensive, person-centered care in compliance with Oregon Administrative Rules (OAR 309-019) , MHACBO standards , and Joint Commission (JACHO) accreditation requirements. This role is ideal for someone who values teamwork, accountability, and direct engagement with patients both on-site and in the community. Available Schedule Shifts Full-time positions are available for one of the following shifts: 10:00 AM
  • 6:30 PM (Saturday
  • Sunday) & 7:00 AM
  • 3:30 PM (Monday
  • Wednesday) Flexibility may occasionally be required for early mornings, evenings, weekends, or community-based activities. Key Responsibilities Direct Support & Case Management Provide clinical case management and recovery support under the supervision of the CRM Supervisor and Clinical Director. Develop individualized recovery plans and help patients establish meaningful short
  • and long-term recovery goals.
Assist patients in identifying and accessing vital resources including housing, employment, education, financial aid, healthcare, and social services. Coordinate care with external providers and systems (child welfare, criminal justice, mental health, medical, and dental). Document case management activities and progress notes in compliance with OARs, MHACBO, and Spark Wellness policy. Provide clinical insight and support as a CADC-R to strengthen recovery planning and interventions. Group Facilitation & Clinical Support Facilitate and co-facilitate group sessions (educational, process, or recovery-oriented) using evidence-based practices. Support Milieu Counselors in maintaining a structured, therapeutic, and recovery-positive environment. Participate in program activities and assist in planning and executing sober recreational events. Transportation & Community Engagement Safely transport patients to and from Spark Wellness facilities, approved housing, community events, and off-site fellowship or 12-step support meetings (AA, NA, SMART Recovery, etc.). Represent Spark Wellness professionally during community-based activities. Conduct home visits or community check-ins when clinically indicated or directed by the CRM Supervisor. Promote patient participation in substance-free community activities to encourage social inclusion and sustainable recovery. Collaboration, Compliance & Professional Conduct Maintain strict professional boundaries, ethical behavior, and confidentiality at all times. Attend weekly team meetings, supervision, and mandatory training sessions. Demonstrate reliability, accountability, and initiative in all aspects of the role. Adhere to all Spark Wellness policies, including those governing ethics, confidentiality, harassment, and a drug-free workplace. Complete documentation and reporting accurately and within required timeframes.
Qualifications Required:
Current and valid CADC-R certification through MHACBO . Eligible for or able to obtain Certified Recovery Mentor (CRM) credential within 60 days of hire . Valid Oregon driver's license with a clean driving record. Ability to pass a DHS Background Check per
OAR 407-007
. Strong communication, active listening, and interpersonal skills. Demonstrated reliability, integrity, and ability to maintain professional boundaries.
Preferred:
Experience working in behavioral health, SUD, or co-occurring treatment environments. Familiarity with Oregon's behavioral health system, OAR 309-019 requirements, and Joint Commission standards. Proficiency with electronic health records (EHRs) and documentation requirements. Prior experience facilitating groups or coordinating patient resources. Work Environment & Physical Requirements Driving required for patient transportation and community engagement. Frequent movement between facility areas, including stair use and short-distance walking. Regular interaction with patients and staff in dynamic treatment environments.
