Job Description

New Wave Home Care is growing and looking for experienced, compassionate Community Liaison to join our team! As a Community Liaison with New Wave, you will have the opportunity to advance your career. Come join us! Job Summary The purpose of this job is to communicate, network, build strong relationships, educate and follow-up with referral sources in Temecula, Murrieta , Riverside and San Bernandino Counties. This position requires a great deal of time to be spent in the community and hospital environment. The Community Liaison will be responsible for organizing and implementing referral development, marketing, and assessment programs to assure awareness and a positive community image. Must be able to work independently, set and track goals and develop an effective community outreach program The Community Liaison will work on a full-time basis to oversee and provide the best continuity of patient care by acting as a liaison among hospitals, doctors, homes and home care agencies.
Community Liaison Home Care Job Responsibilities and Duties:
  • Establish and maintain good relationships with hospitals, nursing services, social services, therapy offices and various other medical and non-medical support services
  • Communicate personally with patient care managers to ensure standards of care are being met and that patient needs are being identified and met on an ongoing basis
  • Recognize short-term and long-term topics that need addressing, and work on finding solutions
  • Lead orientation and in-service training for personnel such as home care intake coordinators
  • Chat with hospitals, doctors, prospective clients and their families about the home care services the agency provides
  • Gather and prepare information in advance of patient transfers; track data related to such transfers and compile information into reports such as statistical reports
  • Keep communication smooth between hospitals and doctors and home care agencies by continually reporting patient progress and therapies received
  • Develop and assess policies for home care, including provisions for meeting federal, state and local regulations as well as professional and licensing standards
  • Visit, revisit, inform and educate between 25 and 30 network referral sources on a weekly schedule.
  • Conduct a minimum of 2 educational presentations/info sessions to groups each month to raise awareness.
  • Assist New Wave Home Care in developing, managing and maintaining referral relationships.
  • Acknowledge all network referral source contacts with appropriate correspondence.
  • Continually research and identify sources for client referrals.
  • Maintain records & provide the Administrator with weekly networking activities and progress report.
  • Provide advice to management on key developments within the service area.
  • Keep up to date on information and issues related to the position, the community, referral networks, and the company.
  • Participate in internal and external professional development activities as appropriate. Community Liaison Home Care Job Requirements and Qualifications
  • Bachelor's degree in Marketing, Business Administration, or related field preferred, but not required.
  • Some previous experience in some sort of medical liaising job (private case management, for example)
  • At least three years of experience in health care marketing management preferably in home care operations.
  • Excellent oral and written communications
  • Bilingual skills (particularly in Spanish) beneficial but not required
  • Strong networking, presentation, and follow-up skills.
  • Excellent communication & listening skills.
  • Self-confident, enthusiastic, strong initiative and motivation, and a desire to succeed.
  • Ability to work independently and handle multiple tasks concurrently.
  • Marketing/Sales experience is highly preferred.
  • Demonstrates good communications skills, negotiation skills, and public relations skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
  • Ability to market aggressively and deal tactfully with customers and the community.
Job Type:
Full-time Pay:
$75,000.00 - $95,000.00 per year
