Job Summary We are seeking a dynamic and passionate Community Liaison & Outreach Coordinator - Developmental Disability and Home Care Services . This role could include developing relationships with DD case managers, service coordinators, hospitals, schools, social workers, families, guardians, and community organizations , while educating referral sources about Veracity's available services. Outreach Coordinator to lead community engagement initiatives and foster meaningful connections with diverse populations. In this paid role, you will develop and implement outreach programs, manage volunteer teams, and collaborate with community partners to promote health, education, and social services. Your energetic approach will inspire community involvement, support underserved populations, and strengthen our organization's presence across various communities. The ideal candidate is a proactive leader with excellent communication skills, experience in grassroots organizing, and a deep commitment to social impact. Duties Key Responsibilities Community Outreach Build relationships with DD service coordinators, case managers, hospitals, schools, social workers, guardians, families, and other community organizations. Introduce Veracity Systems and explain available services. Attend community meetings, provider events, and networking opportunities. Referral Development Identify potential participants who may need home care or developmental disability services. Maintain relationships with referral sources. Follow up on referrals and communicate the status to the appropriate Veracity staff. Marketing Develop and distribute professional marketing materials. Promote Veracity's services through community events, presentations, and appropriate online platforms. Help maintain a positive and professional image of the company. Provider/Agency Relationships Establish relationships with

personnel, DD agencies, healthcare providers, schools, and other service organizations. Keep referral partners informed about Veracity's service availability and areas served. Participant & Family Communication Provide general information about available services to individuals and families. Help families understand the referral and intake process. Direct clinical, medical, eligibility, or regulatory questions to the appropriate qualified staff. Referral Tracking Maintain a database or log of referrals, contacts, follow-ups, and outcomes. Follow up with referral sources in a timely manner. Report marketing and referral activity to management. Community Networking Develop connections with organizations serving individuals with developmental disabilities and people needing home care. Identify opportunities for partnerships and community collaboration. Compliance & Professional Conduct Follow Veracity Systems policies and applicable DD/home-care requirements. Protect participant confidentiality. Do not provide direct care or represent yourself as a qualified clinical provider unless specifically authorized and qualified to do so. Ensure marketing statements about Veracity's services are accurate and approved. Pay:

$20.00 - $40.28 per hour Expected hours: 25.0 - 40.0 per week
