Job Description

LifeStream Services, Inc. Transportation Navigator SUMMARY Responsible for developing community relationships to provide transportation resources to individuals in East Central Indiana through the Rural Health Transformation Program. This role includes developing a "Ride East Central Indiana" resource database of available transportation options in East Central Indiana. This Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $206,927,896.80 with 100 percent funded by CMS/HHS. ESSENTIAL DUTIES AND RESPONSIBILITIES

Other duties may be assigned. Duties and responsibilities include the following. Transportation Information to Community
  • Develops and maintains the Ride East Central Indiana resource database of transportation options available in East Central Indiana.
  • Delivers presentations to individuals and groups including health clinics, physician offices, and hospitals within the community regarding transportation options.
  • Holds trainings to educate community members on how to use resources.
  • Staff health fairs to provide information about Ride East Central Indiana to the community. Direct Services to Individuals
  • Collect information on the individual's transportation needs according to program guidelines
  • Follow agency and program documentation standards to document all activity within the appropriate timeframes
  • Connect individuals to transportation resources and services and provide support with navigating the transportation system
  • Distribute, track and report patient transportation vouchers from the Rural Health Transformation Program
  • Provide assistance with calls and walk-ins
  • Maintain confidentially and follow HIPAA guidelines
  • Follow agency customer service standards Advocacy
  • Advocate on behalf of individuals to ensure access to transportation services
  • Maintain relationships with transportation resources and have working knowledge of services available in the community
  • Monitor access to transportation services so that individuals can access care, attend scheduled appointments and reach resources that impact social drivers of health
QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE
High school diploma/GED with one (1) or two (2) years' experience and/or training in a related field; or an equivalent combination of education and experience. Associate's degree in human services or related field preferred. Must be proficient in public speaking.
MATHEMATICAL SKILLS
Ability to apply basic mathematics, algebra, and geometry.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving concrete variables in standardized situations. Must possess conflict resolution skills and ability to develop positive working relationships with a wide range of constituencies.
COMPUTER SKILLS
To do this job successfully, an individual must have working knowledge of Microsoft applications, and to be able to quickly learn software programs for various home and community based programs.
CERTIFICATES, LICENSES, REGISTRATIONS
Must possess a valid driver's license. Must be covered, maintain coverage and provide documentation of current automobile insurance meeting LifeStream's amount requirement. Must be able to travel throughout the LifeStream service area independently and beyond the service area on occasion. Must complete trainings to remain in compliance with program requirements.
Pay:
$24.00 per hour Expected hours: 40.0 per week
