Job Description

Case manager

•Housing Services (Danielson, CT) United Services is seeking a compassionate and dedicated supportive Case Manager to provide assessment, advocacy, and case management services to individuals participating in our supportive housing program. This program serves individuals who have experienced homelessness and are living with disabilities, including mental health disorders or substance use disorders. provides person-centered services that promote housing stability, self-sufficiency, and overall well-being.

•Spend up to 70% less time on paperwork with documentation support technology designed to help you focus more on care. The supportive housing programs follow the Housing First model and provide person-centered case management services that meet individuals where they are, helping them build stability, strengthen independence, and achieve their personal goals.

Location:

42 Maple St, Danielson, CT What You Will Do (Day-to-Day): Provide assessments for assigned clients. Develop, monitor, and update Community Service Plans in partnership with clients. Build and maintain positive, professional relationships with clients, referral sources, community partners, and service providers. Coordinate and facilitate access to community resources and support services. Initiate and monitor client participation in agreed-upon services to ensure continuity and appropriateness of care. Provide individual and group counseling, education, support, transportation, and daily living skills development services. Advocate for clients and support continuity of care across providers and service systems. Assist clients in maintaining housing stability and achieving greater independence. Maintain accurate, timely, and complete records, documentation, and reports. Conduct daily interactions and services with professionalism, respect, and a person-centered approach.

Schedule:

Full-time, Monday through Friday, 8:00 a.m. through 4:30 p.m.

Minimum Qualifications:

Bachelor's degree in a relevant human services field;

Salary Range:

$ 19.53

•$22.10 per hour (depending on experience)

Bilingual Spanish/English Fluency:

$20.52

•$23.22 per hour (depending on experience) or Associate's degree in a relevant human services field plus two (2) years of relevant experience required; or High school diploma or equivalent plus four (4) years of relevant experience required.

Salary Range:

$19.05 to $21.56 per hour

Bilingual Spanish/English Fluency:

$20.02

•$22.65 per hour (depending on experience)

Work Location :

Because our clients rely on face-to-face-support to build trust, this position requires employees to be fully present in the community and in the office.
