Job Description
A Case Facilitator I - Adult operates under supervision of the assigned Area Director or designated
QBHP/LP. A
member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to adult consumers. Activities of Daily Living (ADL) - synonymous with term "skill building," refers to the identification and development of skills/abilities related to independent living. Skill acquisition should be objectively demonstrated (action, behavior). Focus is typically on the present and is interpersonal in nature.Education:
High school diploma or equivalent.Experience:
Must meet OBHP requirements = Two years' experience in mental health or community services preferred.Licenses:
Must possess a valid stste issued Driver's License.Shift:
