Job Summary The Community Support Specialist is responsible for providing direct support and advocacy services to individuals receiving mental health treatment. This position plays a key role in treatment planning and offers assistance with essential community-based needs including education, housing, food security, and linkage to other social services. The Community Support Specialist works collaboratively with clinical staff and community partners to help individuals achieve their treatment goals and maintain stability in the community.

Full-Time Essential Duties and Responsibilities
- Assist clinical team in the development, implementation, and review of individualized treatment plans.
- Provide community-based support services including school advocacy, assistance with housing, food, and medical needs.
- Link individuals and families to appropriate community resources and coordinate service referrals.
- Support clients in building life skills, accessing transportation, and participating in educational or vocational activities.
- Maintain accurate, timely, and confidential documentation of services provided.
- Attend interdisciplinary team meetings and contribute to case coordination.
- Conduct home visits and community outreach to ensure client engagement and support.
- Respond to client crises under the guidance of clinical staff or supervisory support.
- Promote client empowerment, recovery, and independence through strengths-based practices. Qualifications
Education:
Bachelor's degree in social work, psychology, or related field, current DMH Community Support Services credential Experience working with individuals with mental health needs or vulnerable populations preferred
- Strong interpersonal and advocacy skills
- Ability to collaborate with multidisciplinary teams and external agencies
- Basic understanding of mental health systems, community resources, and crisis intervention
- Valid driver's license and reliable transportation required Must pass background check and any applicable registry screenings
- CPR and First Aid certification (or ability to obtain within 30 days) Work Environment Work is performed in office, home, and community-based settings.
The role may involve travel to client homes, schools, and other service locations. Flexible scheduling may be required to meet client needs.
