Job Description

This vital role involves providing comprehensive assistance to promote independence, safety, and well-being within home and community settings. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts. Monitor and observe patients' health status by taking vital signs, administering medications, and updating care plans in accordance with HIPAA regulations. Prepare nutritious meals tailored to dietary needs, ensuring proper food safety and hygiene standards are maintained. Support clients with personal transportation needs for appointments, errands, or community engagement activities. Maintain a safe environment by performing light cleaning duties, ensuring hygiene standards are met in accordance with health protocols. Document patient observations, progress notes, and incident reports accurately to ensure seamless communication within care teams. Provide emotional support and companionship, especially for individuals with dementia, Alzheimer's, autism spectrum disorder, or other developmental disabilities. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient care techniques including medication administration, patient monitoring, and first aid/CPR certification. Familiarity with dementia care, memory care strategies, hospice care practices, and neurodevelopmental disorders support. Ability to perform manual handling tasks such as heavy lifting and patient transfers safely and effectively. Excellent communication skills for engaging with clients, families, and healthcare professionals while respecting privacy laws like HIPAA. Experience working with individuals with intellectual disabilities or developmental disabilities is highly valued. Knowledge of care plans development and implementation tailored to individual needs across geriatrics and pediatric populations.
