Job Overview Join our dedicated team as a Family Home Provider (FHP) and Caregiver, where you will play a vital role in creating a safe, nurturing, and supportive home environment for individuals with diverse needs in your home. This paid position offers an exciting opportunity to make a meaningful difference in the lives of residents by providing compassionate personal care, assistance with daily activities, and implementing individualized care plans. Your energetic and patient approach will help foster independence and well-being while ensuring residents' rights and safety are prioritized at all times. Duties Provide comprehensive personal care assistance, including help with bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques. Assist residents with Activities of Daily Living (ADLs) like eating, toileting, and personal hygiene to promote comfort and dignity. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals. Administer medications accurately following prescribed care plans while maintaining strict adherence to HIPAA regulations for patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Support residents' mental and emotional well-being through engaging activities, effective communication, and compassionate caregiving. Maintain a clean, safe living environment by performing regular cleaning duties and ensuring compliance with health standards. Document all care activities meticulously in accordance with facility protocols and state regulations. Support residents during medical appointments or community outings using appropriate transportation assistance methods. Follow all safety procedures including CPR, First Aid certification requirements, and emergency response protocols to ensure resident safety at all times. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments with individuals who have developmental disabilities or neurodevelopmental disorders such as autism or dementia. Strong knowledge of patient observation techniques, medication administration, vital signs monitoring, and dementia or Alzheimer's care practices. Ability to perform manual handling tasks including heavy lifting and patient transfers using equipment like Hoyer lifts safely. Excellent communication skills for effective client interaction and collaboration with healthcare teams while respecting residents' rights. Familiarity with care plans development and implementation tailored to individual needs for long-term or hospice care settings. Competence in maintaining HIPAA compliance by safeguarding sensitive health information diligently. Experience assisting with ADLs such as grooming, dressing, eating, and mobility support within home & community care settings. Knowledge of emergency procedures including CPR certification and first aid response to handle urgent situations confidently.
