Job Description

Sevita is seeking a compassionate In Home Caregiver to support individuals in their homes with daily living activities. Responsibilities include assisting with personal care, meal preparation, medication reminders, light housekeeping, transportation, and companionship while following individualized care plans. Responsibilities Provide in-home assistance with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and hygiene Support safe mobility, transfers, and positioning using proper body mechanics and any prescribed equipment Prepare simple, nutritious meals and assist with feeding as needed, following dietary needs and preferences Perform light housekeeping such as laundry, tidying, and cleaning to maintain a safe, organized home environment Offer companionship, conversation, and emotional support to reduce isolation and promote quality of life Follow individualized care plans and document care provided, observations, and changes in condition Monitor and report changes in physical health, mood, or behavior to supervisors or care coordinators in a timely manner Provide medication reminders in accordance with company policies and applicable regulations (no medication administration unless specifically authorized) Accompany individuals to appointments, errands, or community activities as needed Practice infection control, safety, and emergency procedures to protect individuals and yourself Collaborate with family members and the broader care team to support person-centered goals Participate in required training and ongoing professional development offered by Sevita Required Skills Personal care and activities of daily living (ADL) support Basic infection control and hygiene practices Safe transfer and mobility assistance Observation and documentation of client condition Basic meal preparation and safe food handling Time management and reliability in a home setting Professional communication with clients, families, and care teams Boundary setting and confidentiality (HIPAA awareness) Basic understanding of dementia, disability, or behavioral health needs Use of adaptive equipment as trained (walkers, wheelchairs, transfer aids) Crisis de-escalation and calm problem-solving Valid driver's license and safe driving for client transport, where required
