Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques. Monitor patients' health by checking vital signs, observing for changes in condition, and reporting findings accurately to healthcare professionals. Administer medication as prescribed following HIPAA guidelines and care plans to ensure safety and compliance. Prepare nutritious meals tailored to dietary restrictions and preferences while maintaining cleanliness in the kitchen area. Support clients with cognitive impairments such as Alzheimer's disease or dementia through specialized memory care techniques and patient observation. Provide transportation assistance for medical appointments, errands, or community outings ensuring safety at all times. Maintain detailed documentation of care provided, patient progress, and any incidents in accordance with home health standards and resident's rights. Qualifications Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as assisted living or nursing homes. Knowledge of patient monitoring techniques, medication administration, first aid, CPR certification, and emergency response procedures. Familiarity with care plans, HIPAA regulations, hospice care principles, and dementia or Alzheimer's care strategies. Ability to perform heavy lifting safely and utilize patient transfer equipment like Hoyer lifts effectively. Strong communication skills for engaging with clients, families, and healthcare teams to ensure comprehensive support. Prior experience working with individuals with autism or developmental disabilities is highly desirable.
