Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote dignity and independence. Medication reminders. Prepare nutritious meals and assist with meal planning to meet dietary requirements. Provide patient observation and monitor changes in condition, reporting any concerns promptly. Help with mobility and transfers using proper techniques such as Hoyer lifts or patient transfer methods to ensure safety. Maintain accurate care plans, document patient progress, and adhere to HIPAA regulations for confidentiality. Offer companionship and engage clients in meaningful activities to enhance mental well-being, including Alzheimer's care, dementia care, or autism support when applicable. Assist with light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment. Provide transportation assistance for medical appointments or community outings as needed. Follow all safety protocols including first aid, CPR certification, manual handling procedures, and emergency response protocols. Qualifications Proven experience working in home health, assisted living facilities, nursing homes, or group homes; experience with geriatrics or hospice care is highly valued. Strong knowledge of patient observation techniques, medication administration, and vital signs monitoring. Familiarity with care for individuals with dementia, Alzheimer's disease, neurodevelopmental disorders, or developmental disabilities. Ability to perform manual handling tasks safely using equipment such as Hoyer lifts or patient transfer methods. Excellent communication skills for client interaction and documentation of care activities. Certification in first aid and CPR is required; additional certifications in HIPAA compliance or hospice care are a plus. Physical ability to lift heavy objects when necessary and perform tasks requiring manual handling skills. Experience working with individuals with intellectual disabilities or neurodevelopmental disorders is preferred.
