ADLs:

Help consumers with essential activities of daily living.

Prepare nutritious meals:

Cook meals and assist with feeding.

Monitor vital signs:

Measure temperature, pulse, respiration, and blood pressure.

Support physical mobility:

Assist consumers with Passive Range of Motion (PROM).

Compliance and Communication Time tracking:

Clock in and out accurately and timely for all shifts.

EVV compliance:

Maintain at least a 90% Electronic Visit Verification compliance rate.

Document services:

Record care accurately on a daily basis.

Submit paperwork:

Turn in timesheets and activity sheets weekly.

Report health changes:

Notify supervisors of condition changes or incidents.

Maintain safety:

Implement proper infection control practices.

Position Requirements Qualifications Age limit:

Must be 18 years or older.

Transportation:

Possess reliable means of transportation to and from work.

Device requirement:

Must have a working cell phone for daily communication and time tracking.

Competency:

Pass testing prescribed by regulatory agencies.

Privacy compliance:

Maintain strict adherence to HIPAA guidelines.

Personal attributes:

Display maturity, stability, compassion, and initiative. Physical Demands and Safety Capabilities Transfer support: Must be physically capable of performing routine and emergency oneperson transfers.

Consumer movement:

Reposition bedbound consumers alone.

Heavy lifting:

Ability to lift up to 50 pounds.

Mobility:

Working Environment Primary location:

Directly inside consumer homes.

Secondary location:

External healthcare or care facilities.

Potential exposure:

Unsanitary conditions, extreme weather, or highcrime areas.
