Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' health by taking vital signs, observing for changes in condition, and reporting findings accurately to healthcare teams. Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary. Support clients with personal transportation for appointments, errands, or community activities to foster independence. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces. Document care activities meticulously, update care plans regularly, and communicate effectively with clients' families and healthcare providers to ensure consistent quality of care. Experience Prior experience working in home health care, assisted living facilities, nursing homes, or group homes is highly preferred. Demonstrated expertise in patient observation, medication administration, and vital signs monitoring. Hands-on experience with Alzheimer's care, dementia care, memory care, hospice care or supporting individuals with neurodevelopmental or developmental disabilities enhances your suitability. Familiarity with patient transfer methods such as Hoyer lifts and patient personal care assistance is essential. Knowledge of resident's rights and HIPAA compliance ensures respectful and confidential treatment of clients. Experience working with individuals with autism or intellectual disabilities supports a diverse range of client needs. Certification in CPR and First Aid is required to respond effectively during emergencies.
