OVERVIEW OF
POSITIONThe Case Manager will provide case management services to reentry job-seekers in collaboration with the program team and external partners. This position is the primary staff responsible for ensuring that the participants successfully complete the program. This full-time position (40 hours/week) will work out of an office in Inglewood.
The ideal candidate is passionate about serving justice-involved individuals, an experienced case manager serving multi-barriered participants, and has strong working relationships with community resources.
: $27.64 - $28.85 per hour based on background, education, and experience. Employee benefits package will include paid time off (e.g., vacation, personal, sick, jury duty, bereavement) and the employer's subsidized contribution to a healthcare plan of the staff's choice.
KEY DUTIES
will include the following: This individual must work successfully with all persons without regard to ethnicity, religion, age, national origin, physical challenge, sexual preference, or gender. Conduct outreach and recruitment activities, including orientations and outreach in the community to recruit participants for the program. Conduct screening/program eligibility processes, using a risk needs assessment. Complete intakes and assessments with qualifying participants. Co-facilitate Cognitive Behavior Intervention (CBI) workshops. Provide lay counseling. Distribute and document milestone incentives/stipends and supportive services to participants. Conduct follow-up contacts to determine whether each participant's needs have been met and outcome of services. Be an enthusiastic and on-going source of support and motivation to participants through one-to-one visits, electronic communication (email, text), and telephonic communication.
Identify, cultivate and track community resources that remove employment barriers for participants, including housing providers, food banks, legal services, etc.
Create, maintain and update complete, accurate, and orderly client files (electronic and hard copies) in confidential storage on a weekly basis. Prepare, maintain and provide program data on a scheduled basis to the Program Manager. In cooperation with other program staff, meet or exceed grant goals.
Maintain a schedule that is consistent with office hours.
Participate in weekly supervision and attend case conferences, meetings and trainings as requested by Program Manager.
Other duties as assigned.
PREFERRED QUALIFICATIONS
Associate's or Bachelor's Degree in Social Work, Child Development, Psychology, Counseling or a related field. Peer Support Specialist Certification and/or Substance Abuse Counselor Certification. 3 years' case management experience with multi-barriered populations. Critical thinking skills. Demonstrated knowledge of local community resources. Familiarity with the criminal justice system. Strong written and spoken English skills. 2 years' experience with data-entry (e.g., CalJOBS; CHAMP). Proficiency in Microsoft Office or similar program; Internet; Email, data entry and periodic reporting Strong work ethic, well-organized, reliable, flexible, and meets deadlines.
