$21-$24/hour, depending on experience About Chosen Hearts Chosen Hearts is a South Los Angeles-based nonprofit providing trauma-informed case management, housing stability support, youth programming, and community-based services. We support individuals and Transitional Age Youth (TAY) experiencing homelessness, housing instability, and other barriers to stability and independence. Position Summary Chosen Hearts is seeking an Interim Housing Case Manager to provide direct case management services to individuals experiencing homelessness at an interim housing site in Los Angeles. The Case Manager will work directly with participants to identify barriers, develop individualized service and housing goals, connect participants to resources, and support progress toward permanent housing. Key Responsibilities Manage an assigned caseload and meet regularly with participants Complete intakes, assessments, service plans, Housing Search Plans, reassessments, and required documentation Assist participants with housing searches, applications, housing readiness, and permanent housing goals Help participants obtain IDs, birth certificates, Social Security cards, income verification, disability verification, benefits, and other required documents Connect participants to healthcare, mental health services, substance-use services, employment, education, transportation, legal services, and community resources Provide referrals, warm handoffs, and follow-up on participant needs and goals Complete accurate and timely case notes and maintain organized participant files Enter and update participant information in HMIS and other required systems Coordinate with housing staff, service providers, and community partners Participate in case conferences, supervision, staff meetings, and required trainings Assist with transition and exit planning Maintain confidentiality, professional boundaries, and participant-centered practices Qualifications Previous experience in case management, homeless services, housing services, outreach, behavioral health, or a related human-services setting Experience working with individuals experiencing homelessness, housing instability, or other high-barrier circumstances Strong written and verbal communication skills Strong organization, documentation, time-management, and follow-through skills Ability to manage multiple participant needs and meet documentation deadlines Ability to work independently while receiving supervision and direction Must have a reliable personal vehicle, valid driver's license, and be able to travel locally as required for client and program needs Preferred Qualifications The following are a plus, but not required : Experience working in interim housing, shelter, transitional housing, or supportive housing Experience using HMIS or another case-management database Familiarity with trauma-informed care, Housing First, harm reduction, or motivational interviewing Knowledge of Los Angeles County homeless-services and community-resource systems
