Case Manager provides case management, guidance, advocacy, coaching, and supportive services to Transitional Age Youth (TAY), ages 18-21, participating in THN Transitional Housing Placement Program for Non-Minor Dependents (THPP-NMD). The Case Manager works directly with Non-Minor Dependents (NMDs) to support the development of independent living skills, educational and employment goals, health and wellness, community connections, stable housing, and overall self-sufficiency. The Case Manager assists each NMD with developing and achieving individualized goals identified through the Transitional Independent Living Plan (TILP), Needs and Services Plan (NSP), and other required service and safety plans. All services must be provided in accordance with California Community Care Licensing requirements, Title 22 regulations, applicable county contractual requirements, and The Hyacinth Network, Inc. policies and procedures.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Case Management and Service Planning Provide ongoing, strengths-based, trauma-informed case management services to assigned NMDs. Meet individually with NMDs and maintain regular face-to-face contact as required by THN, Community Care Licensing, and applicable county contracts. Assess each NMD's strengths, needs, barriers, goals, and available support systems. Assist with the development, implementation, monitoring, and updating of each
NMD's:
Transitional Independent Living Plan (TILP) Needs and Services Plan (NSP) Shared Living Agreement (SLA) Individualized Risk Screening Individualized Safety Plan Other required individualized service plans Monitor progress toward established goals and assist NMDs in identifying and overcoming barriers. Coordinate services and communicate with county social workers, probation officers, service providers, educational institutions, healthcare providers, and other members of the NMD's support team as appropriate. Participate in case conferences, Child and Family Team meetings, placement meetings, and other multidisciplinary meetings when required. Education and Employment Assist NMDs with identifying and pursuing educational opportunities, including high school completion, GED programs, community college, vocational programs, certificate programs, and four-year colleges or universities. Assist NMDs with school enrollment and connecting to financial aid, tutoring, campus support programs, and other educational resources. Provide job-readiness assistance, including: Resume development Job searches Job applications Interview preparation Workplace expectations Employment retention skills Connect NMDs with workforce development programs, internships, vocational training, apprenticeships, and employment opportunities.
MINIMUM QUALIFICATIONS
Bachelor's degree (BA/BS) in Social Work, Psychology, Sociology, Counseling, Human Services, Child Development, Criminal Justice, or a related social or behavioral science field. A minimum of two years of experience working directly with Transitional Age Youth, foster youth, Non-Minor Dependents, or other vulnerable youth or young adult populations is preferred. Experience working with youth or young adults who are pregnant or parenting, experiencing mental health challenges, involved with the child welfare or probation system, experiencing housing instability, or requiring additional behavioral or social-emotional support is strongly preferred. Knowledge of trauma-informed care, positive youth development, independent living services, and community resources is preferred. Ability to understand and follow agency policies, procedures, regulatory requirements, and contractual requirements. Ability to prepare clear, timely, factual, and objective case documentation. Strong interpersonal, written, and verbal communication skills. Strong organizational, time-management, problem-solving, and crisis-response skills. Ability to establish supportive professional relationships with NMDs while maintaining appropriate boundaries. Ability to work independently and collaboratively as part of a multidisciplinary team.
