Job Description

Job Summary We are seeking a compassionate and highly organized Program Advisor/Case Manager to support Veterans through a field-based Veteran Independence Program in Riverside County, CA. This direct hire opportunity is ideal for a human services, mental health, disability services, or case management professional who is passionate about helping Veterans maintain independence, access community resources, and improve quality of life at home. In this role, you will partner with Veterans, care employees, fiscal management services, local VA contacts, and community organizations to coordinate individualized care plans, manage self-directed budgets, and ensure services are delivered in compliance with program requirements. Key Responsibilities
  • Maintain an active caseload of Veterans participating in the Veteran Directed Care Program, ensuring timely support, follow-up, and documentation.
  • Conduct activities of daily living assessments, initial intakes, quarterly home visits, and ongoing service reviews across Riverside County.
  • Assist Veterans with developing, documenting, and implementing self-directed spending plans and support plans based on individual needs.
  • Guide Veterans through hiring, training, supervising, and terminating care employees, including completion of required paperwork.
  • Coordinate with Fiscal Management Services to review timesheets, monitor billing, track expenditures, and ensure care plans remain within budget.
  • Collaborate with VA contacts, community partners, and program stakeholders while maintaining accurate records in compliance with HIPAA requirements. Compensation and Benefits
  • Pay rate: $24.00 to $25.00 per hour.
  • Job type: Direct hire.
  • Schedule:
    Full-time, 40 hours per week, non-exempt.
  • Work arrangement: Field-based and hybrid role serving Riverside County, CA.
  • Travel requirement: Up to 60% local and regional travel for home visits and direct field services. Required Qualifications and Skills
  • Manage a caseload of Veterans receiving services through a self-directed care program.
  • Conduct in-person assessments, home visits, and service planning activities.
  • Support Veteran-directed budgets, care plans, employee paperwork, and fiscal coordination.
  • Maintain accurate case documentation and collaborate with VA and community partners.
  • Bachelor's degree from an accredited college or university.
  • At least 2 years of experience working with individuals with disabilities, older adults, Veterans, or an equivalent combination of education and experience.
  • Valid California Driver's License, reliable transportation, and current auto insurance.
  • Working knowledge of Independent Living philosophy, the Americans with Disabilities Act, the Rehabilitation Act, and the Fair Housing Act.
  • Strong computer skills, including MS Windows, Microsoft Office, Google Suite, and database management.
  • Ability to manage budgets, meet deadlines, prioritize tasks, and work independently in both office and field-based settings.
  • Strong written and verbal communication skills with the ability to interact respectfully with Veterans, families, team members, and community partners.
  • High level of professionalism, ethics, accountability, confidentiality, and attention to detail.
  • Ability to travel frequently, enter and exit a vehicle regularly, remain stationary while driving or working at a desk, and occasionally lift or move up to 10 pounds. Preferred Qualifications
  • Bilingual English and Spanish communication skills.
  • Experience in Veteran services, disability services, independent living programs, case management, social services, or community-based nonprofit programs.
  • Familiarity with Veteran Directed Care, home and community-based services, or self-directed care models.
