Job Description

Job Overview The Black Infant Health (BIH) Program is a specialized Maternal, Child, Adolescent Health (MCAH) program under California Department of Public Health (CDPH) in the Local Health Jurisdiction of San Joaquin County. The focus is to ensure women in California are health before, during, and after pregnancy; ensure all infants are born healthy and thrive in their first year of life; and reduce infant mortality with a focus on reducing disparities. This project is implemented in a hybrid structure with San Joaquin County Public Health Services (PHS). Duties Provides BIH participants with social service case management to include client assessments, Individualized Client Plan (ICP) and enhanced case management Participates in Case conferencing, referrals to services outside of BIH, periodic prenatal and postpartum assessments Collaborate closely with healthcare providers, social workers, and community agencies to coordinate care delivery and ensure continuity of services Gathers and assembles program information for BIH client outreach and community activities planning, coordinating, implementing, and conducting educational opportunities for community-based events, participate in health fairs and presentations for pregnant and parenting people. Coordinate participant transportaion for program related activities Assist clients with navigating complex healthcare systems, including managed care plans and utilization review processes Requirements Bachelor's Degree or higher Case management experience Group facilitation Experience Have experience serving Black parenting/birthing mothers Proven ability to develop and maintain strong relationships with a diverse group of stakeholders Experience translating materials in "community language" Demonstrated commitment to high standards and a history of achieving results despite challenges Passion for serving the community and a deep understanding of trauma-informed care Critical thinking and problem-solving skills How to
