Full-time, Non-exempt SUMMARY The Case Manager delivers housing-focused, person-centered services consistent with program requirements. This role supports individuals experiencing homelessness in achieving and maintaining permanent housing, income stability, and improved health outcomes. Services are delivered in compliance with all applicable federal, state, and local regulations, including HUD regulations, CoC standards, and HMIS data quality requirements.

CORE RESPONSIBILITIES

Housing-Focused Case Management Provide intensive, housing-focused case management consistent with trauma-informed models. Support rapid placement into permanent housing and long-term housing retention. Develop, implement, and regularly update individualized Housing Stability Plans. Maintain a caseload aligned with program standards; intensive case management caseloads are intended to be capped at twenty-five (25) members, subject to funding and program requirements. HUD & CoC Compliance Ensure compliance with HUD program regulations, including but not limited to eligibility determination, documentation standards, and program participation requirements. Conduct and document required assessments, including Coordinated Entry (CES) participation and VI-SPDAT or other approved tools. Maintain full compliance with HMIS data entry requirements, including real-time or near real-time data entry and adherence to data quality standards (accuracy, completeness, timeliness). Support annual and ongoing program monitoring, audits, and reporting requirements. Documentation & Data Quality Maintain complete, accurate, and timely documentation in HMIS and agency systems in accordance with HUD Data Standards. Track and report on key performance indicators, including housing placements, retention rates, income growth, and service engagement. Prepare reports and documentation for internal leadership, funders, and auditors as required. Care Coordination & System Collaboration Coordinate services with CoC partners, including housing providers, healthcare systems, behavioral health providers, and public agencies. Participate in Coordinated Entry processes, case conferencing, and community-wide housing prioritization efforts. Facilitate referrals and ensure effective service linkage, including "warm hand-offs" to partner agencies. Member Support & Advocacy Assist members in accessing mainstream benefits (SSI, SSDI, Medi-Cal, CalFresh) and community resources. Provide tenancy support, including landlord mediation, lease compliance education, and stabilization services. Advocate for members within housing systems and community networks. Crisis Response & Community Standards Provide crisis intervention and de-escalation in alignment with best practices. Maintain a safe, respectful, and inclusive environment in compliance with organizational and public health guidelines. Respond to community inquiries and support outreach and engagement activities. Program Support & Operations Participate in program development, quality improvement initiatives, and member feedback integration. Demonstrate teamwork by supporting operational activities as reasonably required. Participate in required meetings, trainings, and professional development aligned with HUD/CoC standards.

PERFORMANCE STANDARDS

(AUDIT-READY) Maintain compliance with all HUD, CoC, and agency program requirements. Achieve and maintain established benchmarks for housing placement and retention. Ensure 95-100% data quality standards in HMIS (timeliness, accuracy, completeness). Meet documentation timelines and audit readiness standards at all times. Demonstrate consistent professional conduct, collaboration, and adherence to policies.

) Ability to communicate effectively in person, electronically, and in writing. Ability to use a computer and HMIS systems for extended periods. Ability to travel locally to accompany members and attend meetings. Ability to manage competing priorities and respond to crisis situations. Qualifications Minimum of one (1) year experience providing direct services, case management, crisis intervention, housing navigation, or supportive services to vulnerable populations. Demonstrated ability to work effectively with individuals experiencing homelessness, poverty, mental health challenges, substance use disorders, trauma, or chronic health conditions. Knowledge of trauma-informed care, harm reduction, motivational interviewing, and client-centered service approaches. Ability to manage a high-volume caseload while maintaining accurate, timely, and organized documentation. Experience developing individualized service plans, conducting assessments, coordinating referrals, and monitoring client progress. Strong verbal and written communication skills with the ability to interact professionally and compassionately with clients, staff, community partners, and stakeholders. Ability to maintain professional boundaries, exercise sound judgment, and handle confidential and sensitive information in compliance with applicable laws and agency policies. Proficiency in Microsoft Office Suite, Google Workspace, electronic case management systems, and general data entry/documentation requirements. Ability to work independently, prioritize competing demands, and function effectively in a fast-paced environment. Reliable transportation, valid California driver's license, and proof of insurance if driving on agency business may be required. Ability to successfully pass all required background checks and comply with agency policies and procedures. Preferred Qualifications Experience working in homeless services, supportive housing, behavioral health, or nonprofit social services. Knowledge of Contra Costa County housing, healthcare, employment, and public benefits systems and resources. Bilingual or multilingual abilities strongly preferred. HMIS experience preferred. CPR/First Aid certification preferred. Experience collaborating within multidisciplinary teams and community partnerships.
