THP Case Manager Position Are you passionate about making a difference in the lives of youth? Do you have a background in social work, psychology, or a related field? Children First FFA invites dedicated professionals to join our team as a THP Case Worker. As a THP Case Worker, you will play a crucial role in providing social work case management to youth placed in the care of our THP Program. About Children First FFA:
Children First FFA is a leading organization committed to creating a nurturing environment for youth in need. Our THP Program focuses on empowering young individuals through comprehensive support, education, and mentorship. We believe in the potential of every child and strive to provide them with the tools they need to succeed in life.
Key Responsibilities:
  • Conduct weekly home visits to assess the well-being of youth under our care
  • Engage in one-on-one interactions with each youth a minimum of twice per month, providing essential emotional support and guidance
  • Coordinate and facilitate medical, dental, mental health, school, and other necessary appointments/services crucial for youth success
  • Develop initial Appraisal, Needs, & Services Plans (ANSP) and update quarterly to ensure personalized and effective support
  • Maintain thorough and timely Special Incident Reports (SIR) for accurate documentation
  • Foster professional communication with service providers and actively participate in meetings and group supervisions
  • Actively seek guidance from supervisors, collaborate with colleagues, and adhere to Mandated Reporter laws
  • Respond to crisis situations effectively, implementing appropriate intervention plans and techniques
  • Participate in an after-hours on-call rotation to provide assistance when needed
Qualifications:
  • Bachelor's degree in social work, psychology, or a related field is required
  • Minimum of 2 years of case management experience is preferred
  • Excellent problem-solving skills, ability to think ahead, and anticipate needs for successful job fulfillment
  • Strong communication skills, both verbal and written, with the ability to express ideas clearly and concisely
  • Proficiency in using various communication methods, including email, text message, telephone, and in-person interactions
Physical Demands:
  • Ability to perform various physical tasks, including standing, walking, sitting, lifting, carrying, pushing, pulling, climbing, stooping, kneeling, bending, reaching, and using hands and wrists
