The Case Manager plays a central role in the day-to-day lives of residents in our long- term residential treatment program. Through consistent presence, meaningful relationships, and individualized support, the Case Manager helps residents build stability, engage in treatment, strengthen connections, and develop the skills and confidence needed for greater independence. This is not a desk-based case management role. The Case Manager is embedded in the milieu and spends significant time alongside residents throughout the program. They get to know their residents well—not only their treatment goals, but how they spend their days, what motivates them, where they struggle, what brings them joy, and what helps them feel connected and successful. The Case Manager supports the everyday foundations of resident-centered care: eating meals, maintaining personal hygiene and living spaces, managing routines and responsibilities, attending groups and activities, connecting with others, and participating in the broader community. They provide encouragement, practical support, accountability, and consistent engagement as residents work through the challenges that can make these seemingly simple tasks difficult. Support may include prompting, coaching, problem-solving, and working alongside a resident in real time to help them initiate and complete tasks, while gradually building greater independence. The Case Manager is not a clinician and does not provide psychotherapy or other clinical treatment. Instead, they serve as a trusted and observant member of the resident's support network and treatment team. Support will be provided through this role during times of distress or need but elevated to clinicians where needed. By being consistently present, they are often among the first to notice when something has changed. The Case Manager develops a holistic understanding of each resident's wellness and functioning, connecting observations across areas of daily life and helping ensure that emerging needs are recognized and appropriately addressed. Qualifications Bachelor's degree in Psychology, Social Work or related field preferred; however, equivalent training and work experience may be considered. Prior experience in case management, residential treatment, mental health, or a related setting preferred. Knowledge and experience in the areas of residential treatment, resident care, mental health, activities of daily living, community resources, and resident support. Ability to develop consistent, trusting relationships with residents while maintaining appropriate professional boundaries. Ability to provide individualized, real-time support, including prompting, coaching, encouragement, modeling, and hands-on assistance as appropriate. Ability to recognize changes in resident functioning and communicate concerns appropriately to members of the treatment team. Ability to collaborate effectively with clinicians, residential staff, nursing, and other members of the interdisciplinary team. Excellent communication, interpersonal, organizational, and customer service skills. Ability to maintain timely, accurate, and objective documentation. Ability to stand or walk during most of the 8 hour shift and a minimum lifting capacity of 30 pounds. Must possess a valid Florida driver's license that meets insurance carrier requirements. Completion of all pre-employment requirements to include TB testing, Drug screen, ACHA Level II Background check ( https://info.flclearinghouse.com/ ), and favorable references/screenings.
