Job Description

We are seeking a knowledgeable, dependable, and highly organized Targeted Case Management Supervisor to lead and support our case management team. The ideal candidate must be responsible, a strong team builder, and a collaborative team player who can manage multiple priorities while promoting accountability and high-quality client services. The TCM Supervisor will supervise case managers, oversee daily program operations, and ensure that all client charts and services comply with Medicaid, insurance, agency, and regulatory standards. Essential Duties and Responsibilities Provide direct supervision, guidance, and support to case managers. Build a positive and collaborative team environment. Monitor employee performance and ensure case managers complete all assigned responsibilities. Review client charts regularly for accuracy, completeness, quality, and compliance. Ensure assessments, service plans, progress notes, authorizations, referrals, and other required documents are completed correctly and on time. Ensure services and documentation meet Medicaid, insurance, contractual, and agency requirements. Conduct routine chart audits and develop corrective action plans when deficiencies are identified. Train and coach staff regarding documentation, service delivery, compliance, and agency procedures. Assist case managers with complex cases, client concerns, and care coordination. Monitor caseloads, productivity, service authorizations, deadlines, and required follow-up. Ensure case managers connect clients with appropriate medical, behavioral health, housing, employment, educational, and community resources. Participate in staff meetings, treatment-team meetings, and quality-improvement activities. Communicate professionally with clients, families, providers, insurance companies, and community partners. Maintain client confidentiality and ensure compliance with HIPAA requirements. Report compliance concerns, incidents, and service-delivery issues to agency leadership. Manage all supervisory and administrative responsibilities in an organized, timely, and professional manner. Perform other duties as assigned. Required Qualifications Master's degree in social work, psychology, counseling, mental health, human services, or a related field. Must be a Florida Certified Supervisor. Previous experience in targeted case management, behavioral health, social services, or a related field. Previous supervisory or leadership experience. Strong knowledge of Medicaid, insurance, documentation, and chart-compliance requirements. Ability to supervise, motivate, coach, and develop a team. Excellent organizational, time-management, and problem-solving skills. Ability to manage multiple responsibilities, deadlines, and priorities. Strong written and verbal communication skills. High level of professionalism, accountability, dependability, and attention to detail. Ability to work independently while also serving as a supportive team player. Proficiency with electronic health records and standard computer applications. Ability to maintain confidentiality and appropriate professional boundaries. Schedule and Compensation Schedule, salary, and benefits will be discussed during the interview process and will be based on qualifications and experience.
Pay:
$28.00 - $30.00 per hour Expected hours: 15.0 - 20.0 per week
