JOB SUMMARY

: The ROC ("Reaching Our Community") Resiliency Support Clubhouse help youth with mental health issues build self-esteem, resiliency and life skills in a fun and safe environment. The Case Manager will work from Mondays through Thursdays from 10:30am to 7:30pm and Fridays from 8:00am to 4:00pm.

Duties and Responsibilities include:

Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the individual's individual recovery plan. Partner with the individual and service providers in the development of the individual service plan. Provides assistance and support in crisis situations. Provides assessment, monitoring, and assistance to individuals. Establishes professional and ethical relationships with individuals, families, and coworkers Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community. Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to Agency guidelines. Other duties as assigned.

Public Student Loan Forgiveness Minimum Qualifications :

Bachelor's degree in Psychology, Social Work, Sociology, Human Services, Criminology, or a related field of study. Skilled in working independently and implementing time management strategies. Computer skills sufficient to learn and utilize the electronic medical records. 1-2 years of professional experience in behavioral health or substance use disorder services is preferred. Criminal background check clearance, official academic transcripts, valid driver's license/ID, drug screen, TB test, and an eligible 7-year Motor Vehicle Report. Bilingual abilities a plus.
