Program Overview:
The Crisis Service and Diagnostic Center is a program designed to offer individual tailored assistance and brief (up to 90-day) stabilization for a maximum of sixteen (16) adults aged 18 and above, who have intellectual or developmental disabilities and/or concurrent IDD/BH diagnoses. These individuals are facing psychiatric challenges and/or significant emotional/behavioral changes that are impacting on their essential functions, potentially jeopardizing their ability to stay in their usual home environment.
Key Responsibilities:
. Conduct mental health assessments, develop individualized treatment plans, and provide crisis intervention services. Coordinate referrals and link clients to housing, aftercare, and community resources. Collaborate with families and multidisciplinary teams to ensure comprehensive care. Maintain thorough documentation and ensure compliance with legal, ethical, and regulatory standards. Support staff development, participate in case reviews, and promote a safe, productive work environment.
Qualifications:
Bachelor's degree in social work, Counseling, Human Services, Psychology, or a related field required . Minimum two (2) years of experience in behavioral health, intellectual and developmental disabilities (IDD), or related services required . Strong computer skills required .
