Direct Services Case Manager

Job Description

Purpose:
Prevents homelessness and promotes housing stability by assessing client needs and eligibility and coordinating financial assistance, in-kind support, and case management services in accordance with organizational guidelines.
Job Duties:
1. Manages incoming EFAP requests and conducts client interviews, assessments, and eligibility screenings. 2. Reviews supporting documentation, prior assistance, and program history to confirm eligibility and compliance with assistance limits. 3. Evaluates the connection between the qualifying crisis, documented financial impact, and assistance requested. 4. Provides short-term case management, goal setting, referrals, and ongoing client support as appropriate. 5. Communicates documentation requirements, eligibility decisions, and next steps clearly and professionally. 6. Administers financial assistance in accordance with organizational policies, employer-specific guidelines, and funding requirements. 7. Ensures required documentation and approvals are complete before payment processing. 8. Maintains complete, accurate, and timely case files, case notes, and supporting records. 9. Updates applicant trackers, fund tracking records, and program databases to reflect current case status, assistance, and outcomes. 10. Maintains confidentiality and protects sensitive client, employer, financial, and organizational information. 11. Escalates complex or unusual cases to the supervisor for review. 12. Provides consistent, professional customer service and appropriate referrals to internal and community resources. 13. Represents the organization professionally and performs additional duties as assigned by the supervisor.
Minimum Qualifications:
Education:
Bachelors degree in related field
Experience:
Minimum one year experience in case management services.
Skills:
Strong interviewing, active listening, and assessment skills. Ability to manage confidential information with discretion and integrity. Proficiency in basic computer applications and data entry; experience with client databases a plus. Excellent communication and interpersonal skills, with the ability to work respectfully and effectively with diverse populations. Strong organizational skills and attention to detail.

Two-way appropriate intercommunication with supervisor to receive necessary information to achieve job responsibilities.

Ability to sit for extended periods; able to bend and lift up to 30 pounds.

Must be able to multi-task in a fast-paced environment; have excellent problem-solving, communication, and interpersonal skills.

Must have a positive attitude and courteous and professional demeanor; leads by example to promote positive employee morale and efficient operations.
Impact:
This position is responsible for the efficient coordination of resources to meet specific client needs. Unprofessional behavior or ineffective management of clients and/or programs could result in loss of funding for the organization.
Pay:
$45,000.00 per year
