  • $45,000 a year 3 hours ago Qualifications Microsoft Excel Microsoft Outlook Passenger vehicle Driver's License Cultural competency Typing Full Job Description GAP Services Case Manager The Gap Services Case Manager provides appropriate case management and supportive gap services to eligible Cuban/Haitian Entrants (CHE) and other ORR-eligible populations through outreach, intake and assessment, referral, and follow up.
The CHE case manager will provide individualized health education and advocacy for the CHE's access to healthcare and necessary referral services to help CHE clients become more socially integrated and culturally acclimated in their new communities. The CHE case manager will provide support to all CHE's for a maximum of 90 days unless a client is especially vulnerable. In the event of a an especially vulnerable client in need of services beyond the 90-day service period, (large family, women-at-risk, elderly clients, CHE's that identify as LGBTQ, CHE's with significant physical or mental disabilities, and other vulnerable populations) the CHE case manager will refer clients to Intensive Case Management (ICM) for continued appropriate services.

  • Refugee & Immigrant Services Reports to Refugee Client Services Manager Located in Savannah, Georgia, with frequent travel to Claxton, Georgia Up to 50% locally Monday through Friday; 8:30AM
  • 5:00PM Annual Compensation $40,000
  • 45,000 Responsibilities Assesses clients for eligibility using Preferred Communities-Gap Services for Cuban/Haitian Entrants guidelines.
Provides translation/interpretation services as needed to English. Provides direct services to/for Inspiritus clients as assigned in a professional, courteous, and caring manner. Conducts outreach, intake and assessment, referral, and follow-up to eligible clients. Conducts two home visits within the 90-day service period, including an initial home visit and safety check. Maintains communication with CHEs every 30 days. Provides referral for a domestic medical screening, tuberculosis screening and any necessary vaccines required for parole applications. Provides training and education related to serving CHEs to community partners. Assesses clients for housing and other immediate emergency needs as necessary. Provides clients with linkages to cash and medical assistance, employment, and social services. Assists clients and sponsors with applications for Social Security, Employment Authorization Documents, driver's licenses, school enrollment, etc. as necessary. Provides vulnerable clients with referrals to Intensive Case Management as necessary.
Qualifications:
Bachelor's degree in a social service or similar field and minimum of two years of previous direct work experience in a culturally diverse environment, or an equivalent combination of education and experience. Ability to organize multiple client-related activities. Ability to balance the needs of the client and the employer. Conviction about the capacity of people to grow and change. Ability to set limits and maintain the helping role of the practitioner Excellent computer skills, accurate typing, and knowledge of Microsoft Word, Excel, and Outlook. Must have the ability to communicate with all persons in a culturally competent manner. Must be sensitive to the service populations cultural and socioeconomic characteristics. Must own a personal multi-passenger vehicle, hold a valid Georgia/Alabama Driver's license, have a good driving record, and have appropriate personal auto insurance. Preferred Education and Experience Reads, writes, and speaks a relevant foreign language: Haitian Creole and/or French. Knowledge of immigration issues. Experience with human services programs. Physical Demands:
Regularly required to talk and hear. Required to stand, walk, sit, use hand to finger, handle or feel objects, tools and controls: and reach with hands and arms. Required to operate personal vehicle. Employees frequently lift and or move up to 10 pounds. Specific vision abilities required by this job include close vision and ability to adjust focus.
