Job Description

Mental Health Kokua seeks a compassionate Case Manager to support adults with serious mental illness in community settings across Hawaii. In this role, you will coordinate individualized service plans, connect clients to housing, healthcare, and benefits, and monitor progress toward recovery and independence. You'll conduct home and community visits, provide crisis intervention and advocacy, and collaborate closely with clinicians, peers, and community partners. Responsibilities Provide community-based case management to adults with serious mental illness Develop, implement, and monitor individualized service and recovery plans Coordinate housing, healthcare, and social service referrals and linkages Conduct regular home and community visits to assess needs and progress Document services, goals, and outcomes in electronic records accurately and timely Collaborate with clinicians, peers, families, and community partners to support recovery Provide advocacy and support in accessing benefits and community resources Assist with crisis intervention and safety planning as needed Promote client independence, community integration, and recovery-oriented care Maintain compliance with agency policies, confidentiality, and regulatory standards Required Skills Case management Mental health assessment Service planning Community resource navigation Crisis intervention Motivational interviewing Electronic health records (EHR) documentation Benefits and entitlement coordination Risk assessment Care coordination
