Job Description
Mental Health Kokua seeks a compassionate Case Manager to support adults with serious mental illness in community settings across Hawaii. In this role, you will coordinate individualized service plans, connect clients to housing, healthcare, and benefits, and monitor progress toward recovery and independence. You'll conduct home and community visits, provide crisis intervention and advocacy, and collaborate closely with clinicians, peers, and community partners.
