Violence Prevention & Reduction (VPR) Program Manager Contract 1099 •Runs until May 31st, 2027 Overview The District 209 Case Manager supports youth participating in the Violence Prevention and Reduction (VPR) Program by providing direct case management, mentoring, and referrals in school and community settings. This role focuses on engaging youth at risk of violence or victimization, connecting them to services, and supporting positive outcomes through trauma-informed, youth-centered care. Key Responsibilities Build trusting relationships with youth in school and community settings Provide one-on-one case management and supportive services Conduct intakes, assessments, and ongoing documentation Connect youth and families to internal and external support services Coordinate referrals to clinical staff and community partners Track participant progress and maintain accurate case notes and data Support incident response, mediation, and conflict resolution efforts Participate in staff meetings, trainings, and partner collaborations Perform other duties as assigned Qualifications Bachelor's degree in social work or related field OR high school diploma with three (3) years of relevant experience 3-5 years of case management or youth development experience Strong interpersonal and communication skills Experience working with youth from diverse backgrounds Familiarity with trauma-informed care and violence prevention preferred Proficiency in Microsoft Office (Word, Excel, PowerPoint) Ability to travel locally to schools and community sites; reliable transportation required Equal Opportunity Employer PLCCA, Inc. is an Equal Employment Opportunity Employer and values diversity and inclusion.
