Criminal background check is required. Should have a BA in counseling, mental health, psychology, or social work. Our growing organization is seeking a well-organized professional to join our team. The ideal candidate will be comfortable performing a wide range of administrative and clerical duties while providing supportive assistance to the organization's clients. The Case Coordinator will troubleshoot problems and engage in professional development, making this the perfect opportunity for an ambitious individual to partner with management and help increase our organization's presence in the community.

Responsibilities:

Work with the client and other health care providers to create a care management plan that identifies the services needed to assist the client on an ongoing basis. Work with a team of medical professionals, social workers and others to assess the client's medical, mental health and social needs, develop appropriate treatment plans and coordinate the delivery of services. Prepare progress notes and reports for submission to the care management team. Assist in preparing case management plans in collaboration with other team members. Participate in client meetings, service planning meetings and case conferences. Participate in education sessions, both internal and external to the agency.
