Case Manager (Full-Time or Part-Time) Rehab Assist is growing, and we're looking for a compassionate, organized, and motivated Case Manager to join our team! For more than 30 years, Rehab Assist has provided Case Management, Guardianship, and Life Care Planning services for individuals with disabilities and older adults. We proudly serve clients of all ages. Our experienced and collaborative team is dedicated to helping every client achieve the highest possible quality of life through advocacy, coordination, and compassionate support. We are currently seeking a Case Manager and are open to hiring either a full-time or part-time team member. Position Overview As a Case Manager, you will work closely with individuals with disabilities and complex medical needs to coordinate services, advocate for client needs, and ensure the highest quality of life. This position offers autonomy, variety, and the opportunity to build meaningful relationships with clients, families, healthcare providers, and community partners. Responsibilities Provide case management services for individuals with disabilities and complex medical needs. Conduct community-based visits in a variety of residential settings. Coordinate medical, social, and community services to meet clients' needs. Advocate for clients and collaborate with healthcare providers, community agencies, family members, and legal professionals. Communication with attorneys and participation in court-related processes as needed in guardianship matters. Maintain accurate documentation and work closely with other team member to provide the best comprehensive care. Qualifications Bachelor's degree in Social Work, Psychology, Human Services, Nursing, or a related field preferred; equivalent education and relevant experience will be considered. Experience in case management, social work, rehabilitation, counseling, healthcare, or a closely related field preferred. Ability to conduct comprehensive assessments, develop individualized care plans, and coordinate services across healthcare providers, community agencies, and support systems. Strong communication and interpersonal skills with the ability to build effective relationships with clients, families, healthcare professionals, and legal representatives. Excellent organizational, time-management, and documentation skills, with the ability to manage multiple cases independently. Critical thinking, problem-solving, and client advocacy skills to navigate complex situations. Knowledge of healthcare systems and community resources in the Chicago metropolitan area is a plus.
