Full-time, $50,350 Monday-Friday, 9:00 AM-5:00 PM Organization Overview Heading Home of South Central Indiana is a regional initiative focused on ending homelessness across six counties: Monroe, Morgan, Lawrence, Owen, Greene, and Martin. Guided by a community-driven plan, Heading Home works to make homelessness rare, brief, and non-repeating by addressing system-level barriers to housing and increasing access to services through strategic partnerships. Position Summary The Housing Stabilization Case Manager will be part of the Streets to Stability initiative, an effort to address unsheltered homelessness through a direct-to-housing model. The position plays a key role in supporting individuals who have experienced unsheltered homelessness to successfully maintain permanent housing. This position is responsible for helping households navigate barriers to housing stability, maintain lease compliance, strengthen income and support connections, and successfully transition from program assistance. Core Responsibilities Housing Stabilization & Retention Provide stabilization case management once individuals are housed in a year-long lease, designed to support housing retention and tenancy success Assist households with gathering documentation necessary for housing placement and ongoing eligibility requirements Reinforce tenant expectations, lease compliance, and successful tenancy behaviors Identify barriers and emerging risks early and intervene proactively Support households in resolving issues related to landlord concerns, lease violations, household conflict, behavioral health needs, income instability, and other risks to housing Assist households in building long-term supports necessary to maintain housing beyond 12-month program assistance Support connection to available benefits, healthcare, behavioral health services, employment, and other community-based resources Caseload Management Manage an assigned caseload using a housing stabilization approach that prioritizes early intervention and frequent, ongoing assessment of housing stability Adjust engagement intensity and intervention strategies based on household stability level and current risk Maintain accurate and timely documentation in Homeless Management Information System (HMIS) and other required databases Participate in regular case conferencing, workgroups, and supervisor check-ins Document stabilization activities, identified risks, interventions, and household progress toward transition readiness Begin transition planning early and support households in identifying realistic post-assistance housing pathways Review and update transition plans regularly based on household progress and changing circumstances Support households in preparing for successful transition from program assistance, including connection to ongoing supports, budgeting, income planning, and housing navigation as needed Coordination & Partnership Coordinate with landlords, property managers, outreach teams, behavioral health providers, healthcare providers, and community partners to support housing stability Support connection to mainstream benefits, healthcare, behavioral health, employment, and other community-based services Preferred Skills & Experience Experience working with individuals experiencing homelessness, housing instability, behavioral health needs, or complex barriers to housing Knowledge of Housing First, Rapid Rehousing, Permanent Supportive Housing, and/or housing stabilization approaches Ability to manage complex caseloads and prioritize competing demands Strong problem-solving, communication, and coordination skills Experience working collaboratively across systems and provider teams Familiarity with HMIS and housing program documentation requirements Minimum Qualifications Bachelor's degree in social work, human services, or related field preferred Relevant experience working in homelessness services, housing programs, behavioral health, outreach, or case management may be substituted for education Lived experience of homelessness or housing instability preferred Valid driver's license and reliable transportation Please submit resume and cover letter to Erin Reynolds-Nylund at erin@headinghomeindiana.org
