Job Description

Overview:
We are seeking a dedicated Case Manager/ Discharge planner to join our amazing team. As a Case Manager
  • Discharge Planner, you will play a crucial role in providing support and guidance to individuals in need.
You will work with the interdisciplinary team to aid in the process of assessments, life-planning, care coordination, evaluation, and advocacy to meet the resident's comprehensive needs through communication and connecting to available resources to promote well-being.
Duties:
The essential function of this position includes but is not limited to the following: Establish and maintain rapport with clients. Manage discharge plans upon completion of services Reviews and monitors daily inpatient census Communicates the needs of the client to a variety of care team members and follows up accordingly Initiates and facilitates Treatment Team Act as the clients advocate as it relates to independent life needs (i.e. medical appointments, FMLA, short term disability) Provide transportation when needed for clients. Participate in training to improve skills and to increase competency and effectiveness. Facilitate transition for residents being discharged. Perform other duties as assigned by Clinical Director or appropriate Team Leader. Ability to work from home with supervisor approval
Skills:
  • Bachelor's Degree in Human Services or a related field required
  • Proven experience in Addiction or Behavioral Health related field preferred.
  • Strong organizational skills with the ability to prioritize tasks effectively.
  • Excellent communication skills for interacting with clients, colleagues, and external partners.
  • Ability to work effectively in a fast-paced environment while maintaining a high level of empathy and professionalism.
