Job Summary:
We are seeking a Recovery Specialist•Case Management to provide case management and recovery support services to individuals detoxing from substances. The Case Manager will assess client needs, provide counseling and treatment/aftercare planning, coordinate community resources, and serve as a liaison between clients, families, programs, and service providers. The ideal candidate will have experience working with individuals affected by substance use and demonstrate strong communication, crisis-management, documentation, and care-coordination skills.
Key Responsibilities:
Manage an individual caseload and conduct intake evaluations. Develop treatment, discharge, aftercare, and safety plans. Provide medication-management support as needed. Assess continuing-care needs and coordinate appropriate placements. Coordinate with families, referral sources, community agencies, and service providers. Arrange transportation for appointments and aftercare needs. Assist with retrieving patient medications from local pharmacies. Complete required documentation accurately and within established timelines. Maintain required Virtual Gateway access and complete disenrollment assessments. Support milieu management, de-escalation, and crisis-management activities. Assist with identifying and locating potential contraband and conducting unit searches when required. Participate in education, group programming, recreation, and wellness activities. Attend staff meetings and supervisory sessions. Provide appropriate resources and information to affected family members when requested. Maintain confidentiality and respond professionally to client needs and concerns. Perform other related duties as assigned.
Requirements:
Bachelor's degree (BA) or equivalent, or Associate's degree in Human Services or a related field preferred. Candidates without a degree may be considered based on relevant experience. Approximately 2-3 years of related experience and/or training in chemical dependency/substance-use services preferred. Knowledge of community resources for continuing care and aftercare placement. Experience working with a substance-abuse treatment population strongly preferred. Certified Addiction Counselor (CAC) certification preferred. Ability to conduct assessments, identify problems, collect information, establish facts, and develop appropriate solutions. Ability to handle difficult or emotional client situations with sensitivity and professionalism. Strong customer-service and interpersonal skills. Ability to maintain confidentiality and respond appropriately to client, regulatory, and service-provider inquiries.
Pay:
