This role involves ensuring clients have access to essential care and services to enhance their quality of life. Following individualized care plans tailored to each client's specific circumstances is a key responsibility. You will coordinate with various service providers to monitor progress. By effectively bridging the gap between clients and the resources available, you will significantly impact their overall well-being and path toward self- sufficiency. Qualifications (Must pass background check, as required by applicable State of Maryland regulations) High School Equivalent (Required) Case management experience preferred Coordinate with service providers to ensure comprehensive care Strong commitment to ethical practice and maintaining client confidentiality Passionate about mental health and committed to providing high-quality care Advocate for clients' needs and rights Dedicated to evolving professional development to include continuing education Ability to collaborate with a multidisciplinary team to enhance client care Excellent communication, writing skills, and interpersonal skills The ability to build rapport with clients and families to provide high-quality care Ability to manage time effectively and complete documentation and administrative tasks accurately Proficient in electronic health record (EHR) documentation Preferred Skills Cultural competency and sensitivity to diverse populations Crisis intervention and de-escalation techniques Trauma- informed care knowledge Strong problem-solving and critical-thinking skills Evidence-based practices, i.e., Cognitive Behavioral Therapy (CBT), Motivational Interviewing Essential Duties and Responsibilities Follow individualized Service Plans (ISPs) in partnership with participants, emphasizing the attainment of self-sufficiency within the designated timeframe, and monitor progress to ensure goals are achieved Maintain records according to agency, state, and federal regulations Maintain accurate, timely, and confidential documentation in accordance with agency, state, and federal regulations. Responsible for a full client caseload Guide clients through the process of identifying and applying for eligible benefits, including emergency cash assistance, Supplemental Nutrition Assistance Program (SNAP), daycare vouchers, and eviction prevention programs. Continue to provide support by monitoring the progress of applications and ensuring follow-up actions are taken to achieve successful outcomes Adhere to our and the State of Maryland Code of Ethics, Code of Conduct, and all personnel policies and procedures Comply with HIPAA, ethical standards, licensing requirements, and organizational policies Continue special training in designated areas Maintain regular communication and conduct necessary visits to assess progress, tackle obstacles, and encourage participant involvement and adherence to the Individualized Service Plan (ISP) Educate clients on available resources and coping tools Ensure individuals, families, and children receive essential community services by advocating effectively and conducting follow-ups to verify support has been provided Maintain up-to-date credentials, certificates, a valid driver's license, and/or acceptable alternative form/s of identification Cultivate a setting that embodies the principles of Trauma-Informed Care (TIC) by ensuring safety, building trust, and providing emotional support throughout all client engagements Conduct strength-based, culturally sensitive assessments for new clients, creating a welcoming atmosphere for diverse backgrounds Engage in cooperative care alongside our team of psychiatrists, nurse practitioners, dietitians, administrative staff, and fellow therapists, ensuring a comprehensive and integrated approach to mental health care Work independently and collaboratively within a multidisciplinary team Ensure case files are precise and current, promptly input data into the database, and uphold data integrity to facilitate accurate reporting and analysis
